issue with skin/loft

Dear all,
is it possible to do the skin/loft operation considering two circle of different diameter along a particular line? Any help is highly appreciated.

thanks for the reply. Actually what i intend to do is the two circles that u have lofted should be along a specified line, not along the line the design modeler shows

would you like to show us some rough sketch that you want to get?

I want to do the 3D model of the attached figure, where both the outer wall and the inner wall are the function of 'x'. My objective is to loft the two circles (marked in red) at two different planes along a particular line (marked with green), so that I can configure the branch portion of the pipe.
If there is other alternative approach to the problem? If it is so, then give some idea about how to do it.
Thanks in advance
