[ICEM] Problem with volume meshin

Dear all,
I am stil have the same problem, as you will see in the pciture attached, there are some holes in the surface of the volume mesh.
This holes appreas on the edge of the domain, I am using Octree and curvature proximity refinement.
Do you have an idea of how overcome this issue?
