radial flow in porous media - top view

Default radial flow in porous media - top view

I am modelling the flow of water from one well to another, separated by a porous media. I know the pressures in both wells, and the axial velocity in well 1. I will be assuming laminar flow. I know the permeability value, and viscous resistance.

Questions I have:
1. How do I assign pressure boundary condition to the 2 small circles (wells)? Fluent does not give me the option.
2. Is it possible to assign axial velocity boundary condition to this model? If so, how?
3. Is it possible to model this kind of a radial flow in top view? I haven't come across any papers that do it this way? They model it in the side-view.

Please help.
