Default Creating a fluid flow through cavity

I want to simulate the flow of a fluid around a complicated geometry. In this case a knee joint which is inside a fixture of similar shape just a little bigger to provide channel for flow all around the joint.
I am facing trouble using the fill option to generate fluid channel. Any suggestions?
