Does anyone ever use ICEM-CFD to generate mesh for fluent ? What format ?

I use only ICEM CfD.. currently ver 5.0. It is easy. In my opinion is much better than gambit, especialy for 3D meshing. If you want to export mesh to FLUENT, you go to OUTPUT->WRITE INPUT->FLUENT_V6 (or_V5) and ICem creates file <name>.msh. Good luck
