Temperature Gradient Issue!

Hi all!

I am solving conduction on a plate which is on the top of the channel with fluid flowing inside it. I have assigned boundary conditions to the inlet and outlet of the plate (according to one-D conduction with heat generation in the solid). But I see huge gradients just one cell adjacent to the outlet of the plate. The boundary condition at the outlet seems to be right but does anybody know what could be the issue?

Does it have to do with looping over cells/faces in the domain?

I have attached a pic showing the contours of temperature distribution in the plate.
