which one are you referring to? there are more than one.

turbulence viscosity ratio limited to... refers to turbulent quantities. possibly your initial condition are quite far from the solution so a limiting is in place to prevent the simulation to diverge.

the backflow means that the fluid is coming into the domain from the outlet, instead of going out. I'd say pressure-related or mesh problems

it is hard to be more specific without knowing the problem and how the simulation has been set up.

i am also facing similar conditions, but the flow profile is coming almost ok, with some overshooting in the velocity.
does it also means some problem in B.C.'s or mesh

My model is a hull with vof model, 2 phase air and water. I set BC following:
1. Define model: 3D, steady, vof, k-epsillon standard wall funtion
2. Phase: primary phase - air , secondary-phase water
3.Operating condition: operating pressure 101325, gravity z = -9.81
4. Boundary condition: air-inlet with velocity inlet, water-inlet with velocity inlet.
air-outlet with pressure outlet gauge pressure 0. water outlet with pressure outlet gauge pressure 0.
5.Solution control: PISO, Body force weighted.
6. Solution initialization: compute from all-zones, reference frame absolute. init.
7. Solve initialize, patch: phase water, value 1, zones to patch fluid, registers to patch hexahedron-r0, patch.

try to initialize from the inlet
