Concentric pipe geometry

Hello Everyone,
Actually i am trying for a simple geometry of a concentric pipe flow. In which both the pipes are drawn in same plane. Inner pipe is smaller one with respect to height and dia. as compare to bigger surrounding one. Here I am trying with inlet at face of the small pipe and outlet at the face of the bigger pipe and remaining portion as wall.
For this geometry flow is returning inside the small tube, and tip of the inside tube working as wall boundary condition. Whereas if we draw a single pipe, one can able see the flow going out from the outlet this is missing in this case.
I need help for this problem, where this inside pipe outlet wall has to be set as interface so flow will distribute through the tip of the small pipe to the bigger pipe.
(To get an idea about geometry i have attached images of the geometry.)
Kindly give me some suggestions to solve this issue.
