(1). Most CFD books should cover the boundary conditions. Check the forum/books section. (2). Try the book "computational methods for fluid dynamics" by joel H. Ferziger and Milovan Peric, Springer publisher, ISBN 3-540-59434-5. It has many sections on the boundary conditions.

The boundary conditions depend on the problem you have to solved. laminar flow? turbulent flow? steady bc? fully developped flow at the inlet? constant speed at the inlet? Is it an incompressible or a compressible flow? What kind of surface is the tube made of? .....
