FEM Grid generator for WEDGES and TETRAHEDRONS

i have a FEA code which works for 2d 3-noded and 6-noded triangles, 2d 4-noded and 9-noded Quads, 3d 8-noded Brick elements. Now I want to add wedges and tetrahedron into it. I want; 1) Any related material where i can find an algorithm for mesh generaton by using wedge and tetrahedron elements and calculation of the local destination array. 2) Any educational(preferably with source code) code which can do it. I will need the source code for making it compatable with my code.
