Loss of Conservation of mass on a Pressure Corection DNS Solver

Hi I have written a DNS solver for incomressible flow, bassend on a varying resolution cartisean grid.

I programmed it with an object oriented aproach around finite volume grid elements.

When I came to validating my code, I chose to use,

(Schafer et al; 1996) "Benchmark computations of laminar flow around a cylinder". Notes on numerical fluid mechanics 52, 547-566

The first test I chose was the 2D unit circle in a curved fluid velocity profile, and Re = 100

Which should have resuled in a nice von Karman shedding vortex wake,

As the Attached Picture Shows, the solver seems to be converging on a steady state solution, furthermore the flow visualisation clearly shows that local conservation of mass is failing.

The calcualted forces seem so be in the right ball park area, but show no signs of oscillation.

Could anyone sugest any reasons this could be happening,
