Sorry about the delayed bug report, but here it is and with test case.

Description:
snappyHexMesh bails out when increasing nCellsBetweenLevels from 2 to 3 in specific case (below)

Could not find point 129196 in the anchorPoints for cell 97678

Does your original mesh obey the 2:1 constraint and did you use consistentRefinement to make your cells to refine obey this constraint as well?#0 Foam::error::printStack(Foam::-Ostream&) in "/home/OpenFOAM/OpenFOAM-1.5.x/lib/linux64GccDPOpt/libOpenFOAM.so"

Thanks. Pushed a change to 1.5.x git to stop it failing. With more than two layers between levels it uses a geometric test which does not work very well for non-cubic cells so it still might not do what you want.

Adding question to this thread
 
Adding question to this thread since it's still warm, although no bug:

I have generated a snappyHexMesh mesh for a customer running OF-1.5. It was generated on 1.5.x. When running checkMesh in 1.5 there are several errors about concaveFaces and warpedFaces et c. These errors does not apear when the mesh is checked in 1.5.x.

Are ther mesh differences, or only tolerance changes between the versions?

There should be little mesh differences. Mesh checking hasn't changed. Rerun with binary writing and see if that helps.
