[mesh manipulation] Problem with refineMesh controls

Default Problem with refineMesh controls

Hi everyone,
I have tried using refineMesh utility to refine mesh. It worked fine.
I have created refineMeshDict in system directory according to http://openfoamwiki.net/index.php/RefineMesh, unfortunately it is not reading the Dict file.

Can somebody help me.
