How to create patches for this problem

Hi
I am trying to solve a problem for heat exchanger. My problem is how to implement the patch as shown in figure How to map all variables calculated from Outlet of first part to inlet of second part ? Would cyclic condition be helpful ? Please let me know Sachin |
