does anybody know or can even state where the formulas in continuityErrs.H come from?

check out this old thread: SimpleFoam and Time Step continuity errors
the continuity error is calculated by div(phi) what gives you the phase flux. The sum of the flux through all faces of a numerical cell should be zero.
The continuity error adds all deviations. I hope this helps.
