Sounds like a problem across shared points (points shared by more than 2 processors) which is why you only see it on some decompositions.

If you manage to get the polyM

If you manage to get the polyMesh to load: following utility will synchronise the points and rewrite them so you can reset the tolerance. Not very tested.
