Default Tri-meshing issues on a Surface in ICEM CFD

Hello All,

I came across a strange meshing problem in ICEM CFD. The mesh generated on a otherwise fine (water-tight geometry) is shown below. Any comment what might be reason and how can I generate the mesh (all tri)?
