Prism mesh generation problem

Hi to all,
I am doing a steam turbine stage (stator-rotor) meshing. My approach is to generate octree mesh then delauny next insert prism elements. While inserting prism elements into delauny tetra mesh,I found in very few areas prism elements failed to insert and tetra exits in that area.This leads to mesh quality issues. I tried mesh curve parametrs,recreating the surface,and refining the mesh in that area.But the problem still exists. anybody help me on this issue.
