Prism layer not completely forming at intersection

Hi

I am trying to mesh a prism layer around a cylinder that is going to have a jet blowing air onto it. There are two symmetry planes along the z-axis cutting through the cylinder on either side. One of the planes is also cutting through the jet.

When trying to make a prism layer around the cylinder in some cases it works fine and no pyramids form, in other cases (when the geometry is modified slightly for different jet spacing) pyramids form and the prism layer seems to fail at certain areas where the cylinder meets the symmetry plane.

There's only a few elements (~20) I'm just frustrated as to why this is happening and can't seem to figure out how to correct the issue. I am creating the prism layers (5) on an octree mesh, smoothing, then transitioning to a delaunay and smoothing again as the initial setup. Turning on auto reduction and turning off refine prism boundary and stop columns doesn't seem to help. I tried generating one layer and splitting it but the same issue happened.
