ICEM-uncovered faces due to refinement

I have a mesh out of hexas with refinement. Between two levels of hexes a layer of cube-shaped polyhedrals is placed. Unfortunately CFX cannot deal with those, so I want to split up the polys. I tried using the "Resolve refinements" function of ICEM, but I get a "This mesh has no couplings" error (the mesh was not generated in ICEM). I also tried the "Fill holes in volume mesh" function of the Delaunay mesher, but I still end up with uncovered faces. Does anyone know how to solve the problem?
