Tri2tet terminated prematurely

Hello Everyone.
For an external flow CFD analysis of an airplane, I have been generating delauney volume in ICEM CFD. Occasionally when I delete volume mesh of robust octree and generate delauney from surface mesh, there is this error tri2tet failed. What is this error and why does it occur occasionally?
Sometimes I just change the maximum size of a surface and this error appears!?
How do I fix this ?

This error shows that is difficulty of converting tri (2d) elements into tets (tetrahedrons i.e. 3d elements)

Definitly sizing will matter alot. so choose it carefully and according to the geometry features.

Thank you for the response Sir!
I fixed the problem. Coarsening the Max Element Sizes deteriorated the minimum element quality. I fixed it by performing 10 iteration of smoothing and then the same number with laplace enabled. I repeated this process until there appeared no further change.
