Issue Faced in CFD Post

Hello everyone, I am working on a CFD analysis of a concentric tube heat exchanger with counter and turbulent steady flow. The fluid I am using is water for both hot and cold. But the outlet temperatures that I got are not logically correct as you can see in the pics. In addition, when I put streamlines for the cold fluid, it does not cover the whole geometry. Could someone help me in that please

. I am using 2021 R1 student version.

Is it a steady state or unsteady (transient) simulation?
