This happens when you create prisms out of hexahedras and merge the vertices on one of the prism edges: the cell definition does not correspond to the one of a prism and is still a hexahedron. Hence the collapsed face. Run a cell check and put all the cells with collapsed faces into a cell set. Use then: CFLIP,CSET,,,PRISM (or CFLIP,CSET,,,COLLAPSE)
