Mesh adaption to identify high gradient elements leads to 0 quality elements. Left handed cells are reported, mesh repair fails as well. How can I sort this issue?

For left handed cells problem, generally, it only exists in 2d structured mesh. You need to set the correct orientation for your mesh block when generate the mesh.

The mesh is 3d and created in ansys meshing and then imported into fluent. It does not have any negative volumes then but when after getting the simulation result i do mesh adaption in fluent to refine in the high gradient cells it results in negative volume and left handed cells, which i am not being able to put right.

So, the key point is the adaption, not the mesh itself. You may have to adjust the adaption parameters.
By the way, could you upload some pictures to describe details of your mesh adpation?

I have attached the picture, sorry the copy pasting and resizing makes the test distorted but you can still make it out. So the solver runs ok with the mesh on the right but there are regions of high gradient where i want to do mesh adaption, taking the values as suggested in fluent tutorials (i.e refine thershold should be 10% of the maximum gradient). Next in the middle you see the regions in the mesh where the refining of the mesh would take place, but when you actually tell fluent to go ahead with refining the mesh in the regions is shows, it creates these regions but negative volumes than appear in the refined mesh.
