Polyhedra mesh is cutting up edges!

The software is basically taking cuts out of edges when agglomerating tet cells into polys. For example, two surfaces meeting at right angles will have a lump out of it some distance down due to a cell approximation not fully extending to the common boundary curve.
I have not yet figured out the exact circumstances (surface mesh size/maximum volume mesh size/growth factor/seed size etc?) that lead to it occuring.
Has anyone here encountered similar issues, investigated them and understand why fluent is cutting lumps out of the domain?
