How do I convert viscous stresses from cartesian to cylindrical?

stress_xy = mu(dv/dx + du/dy) stress_r(theta) = ??

There are plenty of books on that.

See e.g. Landau Lifshitz, FLuid Dynamics; or some other books on hydrodyanmics from your (public) library... it would be too much to write these down here.
