Question calculation of local h for concentric tube HX

I am simulating concentric tube HX with two fluids on either side. I know mass flow and temperature. I want to find local h variations along the flow on both sides. How i should define boudary conditions for wall between two fluids. I have defined fluids properties thr. polynomial governing equations as fn of temperature.
