Normally are hexahedral and polyhedral meshes classified into unstructured mesh?

Yes, hex and poly meshes are often included in the unstructured category. The exception is when a hex mesh is actually an IxJxK grid (aka mapped mesh) - that's a structured grid (or mesh).
