Shell refinement 99 Iterations

Hello Everyone,
I am using of5x and Ubuntu 17.1. I am doing multiregionmeshing using SHM. I used different blockMesh and refinement combinations to get required mesh. I am able to do meshing. However, sometimes "Shell refinement iterations" reaches to 99 and sometimes it takes only a few iterations. What could be the possible reason for too many iterations.
