Bubble injection in tundish using twoPhaseEulerFoam

Trying to inject bubbles in water tank field using twoPhaseEulerFoam.

My geometry comprises bubble inlet at bottom, atmospheric outlet at top and rest all walls. bubble inlet is small in comparison to water tank.

, but I am getting unexpected high velocities at some regions (specifically near to top outlet), and solver crashes abruptly. I tried both ESI and CFD direct version of openfoam. Please help.
