I wangt to generate unstructured block, when initializing the block, it displays the: The surface triangulation could not be recovered. I don't know what the sentence means?

My initial guess would be the surface triangulation is of really poor quality or invalid. I have some other thoughts but it would be helpful if you can post more details and/or a picture of what the block looks l like.
