When a wall has some motion normal to the wall then streamlines can hit the wall. Just have a play with it and you will see. I do not feel like writing a mathematical proof of it as it is obvious...

It is because you are using velocity in the stationary frame in the rotating section. Use the variable "Velocity in stationary frame" and it won't do that.
