can some one please explain steps to take in resolving highly skewed element in a mesh?

go back to the mesher, correct some parameters, remesh and check cells quality

May be you are meshing a geometry with complicated surfaces. You should decompose the domain so as to limit skewness to as small area as possible and use structured meshing. Skewness arises when the meshes get deformed near the complicated surfaces.

Check in the mesher what is the cause of the skewness (typical: small edges, small angles etc...)
