Default Problem in Generation of Delaunay Mesh Generation

I generted Octree mesh and deleted volume mesh and tried to generate Delaunay mesh. But it pops out of my domain.
I have tried smoothing surface mesh and changing global element seed size of mesh. But it did not solve the problem.
Any ideas how can I solve the problem?
