I guess you know that the quality of the tetrahedral volume elements is heavily dependent on the surface triangle elements.

I looked at your geometry in a pre-release of SymLab v2 (http://www.symscape.com/fourth-openfoam-workshop-2009-abstract) and found some small edges and sliver faces that might be causing Gambit...
