The mesh checker reports 12 op

The mesh checker reports 12 open cells found. Max cell openness: 2.8744 and Failed 3 mesh geometry checks even before you try coupling the mesh interfaces. This is not good - you should make sure...

Look at ~/.OpenFOAM-1.3/contro

Look at ~/.OpenFOAM-1.3/controlDict

If you haven't got that, it will live in the .OpenFOAM-1.3/controlDict directory of your installation.

For some reason, your patch merging has failed. Try switching on debug for:

polyTopoChanger
polyMesh
enrichedPatch
slidingInterface

and see what it says.
