Just a little thought: some of the mesh-manipulation utilities (not all) honor the writePrecision variable in the controlDict. So when these utilities write out node positions they clip everything...

The problem could be the following:
in the controlDict there is a writePrecision set to 6. This is sufficient for data, but not for mesh information (look at the tolerance in your error message)....
