Nozzle Mesh generation in ICEM CFD

I want to generate mesh for a nozzle:
1. Is it necessary to create solid surfaces to impose input and output boundary conditions ? my model is just as hollow 2. Is it necessary to create a solid surface to impose pressure far field boundary condition? and usually how much is the distance from nozzle outlet to pressure far field boundary?
