There is no universal answer, it depends on what you are doing. But things which can help include:
* Mesh smoothing
* Breaking geometry up into blocks to guide the meshing
* Removing unnecessary areas of yukky mesh
* Mesh refinement in problem areas
* Adjusting inflation settings in problem areas
* Changing meshing approach - tets, inflations layers and hexs all are good for different types of mesh
* Manually moving problem nodes
* ICEM has many tools to remesh problem areas

Can you name an example of problem areas in pipe flow, and heat transfer problems?

I can think of many problem areas but it makes no sense to post common grid problem with the risk that they just do not fit your problem.

If you have a specific problem, you should ask a specific question.......
