Error in using UDF

Default Error in using UDF

Dear all,
I am working on a 2-D modeling of slip flow. When I want to set my boundary condition through using UDF files ( Slip velocity and temperature jump at wall) I face this error:

Error:
FLUENT received fatal signal (ACCESS_VIOLATION)
1. Note exact events leading to error.
2. Save case/data under new name.
3. Exit program and restart to continue.
4. Report error to your distributor.
Error Object: ()

I have saved my case under new name also I have changed my mesh. What can this error mean? How can I fix it?
