Default automatic meshing problem

Hi, i have tried to code an automatic mesher which outputs a gambit neutral file. The file can be read into gambit but at some faces, there seems to be extra walls that are there especially between brick and tetrahedral elements.

not sure if anyone can help, just trying my luck. i suspect that it is due to my node assignment in the element connectivity portion of the neutral file.

thanks
