[blockMesh] local mesh refinement by regions

Default local mesh refinement by regions

Hi everybody,
I want to use local mesh refinement for an axisymmetric case, with using cellsets and refinement, but in the axisymmetric boundary, cells are not refined as other cells, Any suggestion?
I searched for the problem I saw a solution to create a 2D mesh and refine its locale then make the mesh axisymmetric by using MakeAxialMesh.
But MakeAxialMesh did not compile for me.
I used foam-extend-4.1

Any solution???
