Radial Distribution of Temperature as a Boundary Condition

Hello Everyone,
I am simulating a case where I need to apply a radial temperature distribution as a boundary condition over a patch (In my case, it's a disc). I have attached an image of the computational domain for a better understanding. The inlet temperature is 298K and the shroud temperature is 378K. The boundary condition of the upstream disc should be a uniform distribution across the disc starting with an initial temperature of 300K. The effective radius (Ro-Rinlet) is 262mm. I have tried codedFixedValue, but that didn't work and giving the temperatures as a scaler list is not workable for my case as it has 7618 faces and 4807 points at the patch.
