Post add option -allRegions

It could also be that you have multiple regions in your time folder.

If that is the case, add the option -allRegions to your reconstruction command.

The problem is not in the .geo file but most probably in how you export the mesh.

Try to write the msh file in version 2 ascii before converting it with gmshToFoam.

It's not mandatory to assign a physical surface to each geometrical surface. Non assigned surfaces are converted into "defaultFaces" patch in OpenFoam.

Try specifying the physical groupements as nasa55 said. But verify you specified them well in tools->visibility...
