Answering my own question

The odd messing was due to refinement being done in only two of the three axes. refineMeshDict must be set to explicitly refine each dimension, such as with this snippet:

coordinateSystem global;...

I'm doing something dumb in setting up a two phase problem, but I can't see it. The model uses a course 3D gradient mesh from blockMesh as a starting point, and then does four layers of refinement...
