Your inlet velocity is set on the direction x ? Shouldn't it be on z (or face normal) ?

I'm running OpenFOAM on cases with backflow occuring at outlets at some times ; it is a physiological flow and therefore cannot be avoided and I cannot change the geometry adding outlet...
