[ANSYS Meshing] Mesh distortion

Hello,

Im trying to mesh a disk, and I used O-grid for this purpose. Everything went well but there is some distorted elements (wedge shape) only at the top and bottom surfaces. quality become very bad due to these elements .I cannot imaging why. can anybody give explanation please
