Check your mesh quality in ICEM itself.

Try to improve mesh quality by smoothing and export it.

Are you using mesh motion? If so this is a FAQ: http://www.cfd-online.com/Wiki/Ansys..._went_wrong.3F

If this is not a mesh motion simulation: The default mesh quality checkers in ICEM do not count negative volume meshes as poor quality, even though the meshes are so bad as to be unusable. You can get this problem by smoothing too much in ICEM. To avoid the problem you need to only do mesh smoothing once in ICEM (unless you know what you are doing to avoid negative volume elements), or use other mesh quality metrics which will pick up the negative volume element.
