continuity convergence issue

hello to you,
i have been modeling a hydrocyclone, however throughout the simulation, my continuity doesnt converge. on the contrary, it keeps growing til unacceptable numbers.
despite trying out various models of turbulence, and etc, there is no variation in this diagram i have attached.
please help me out
any swift help is highly appreciated
