Using soloution results for defining BC.

Hi all;
I am simulating flow within a plate heat exchanger . For this purpose , initially , i computed temperature distribution on plate using the fluent software(kind of plates is stainless steel and two dimensional). Now, i want to consider this plate as boundary for simulation of flow passing on it(3 dimenensional) . Is there a method that i export the temperature distribution information of plate to this boundary(mesh sizing for these two cases is'nt same and considered boundary for flow simulation is the part of original plate)?
